The following are the Register Numbers of candidates who are found provisionally eligible to be included in the Ranked List for the Post of JUNIOR HEALTH INSPECTOR GRADE II on Rs.4000-6090 (Pre-revised) in HEALTH SERVICES DEPARTMENT in KOTTAYAM DISTRICT on the basis of the common OMR Test for Kasaragod District also held on 18/11/2006 (Cat. No.222/05) published in the Gazette dated 31/12/2005.

The Register Numbers are arranged in their numerical sequence and the arrangement does not in any way indicate their respective rank on the basis of the OMR Test.

All candidates who have obtained Sanitory Inspectors Qualification from Kerala State have been included in the List I irrespective of their marks in the OMR Test held on 18/11/2006.

The candidates who have secured 63 marks and above in the OMR Test have been included in the List II (main list) of the Probability List. The marks have been lowered to the extent necessary in respect of candidates included in the Supplementary Lists.

Note 1. Inclusion of the register numbers in this list is purely provisional subject to scrutiny and admission of applications on absolute basis. Inclusion of register number in this Probability List does not confer any right on the candidates to be included in the Ranked list.

Note 2. The Probability list is prepared based on the details furnished by the candidates in the application and subsequent claims made will not be entertained at any stage.

Note 3. Candidates whose register numbers are included in the Probability List are directed to produce Self attested copies of documents to prove their claims made in the application ie. Qualification, date of birth, community, and other benefits, if any, to which they are entitled, at the time of Certificate Verification. The register numbers of those candidates who do not appear in person for certificate verification will not be considered at the time of preparation of Ranked List.

The candidates included in the Probability List (excluding PH candidates) shall appear for Certificate Verification at the KPSC DISTRICT OFFICE, KOTTAYAM with the original and self-attested copies of Certificates to prove their date of birth, Educational qualifications and community on the date and time mentioned below.

For the timely publication of the Ranked List further time will not be granted for the production of the documents other than the time specified in the time table. Therefore the candidates should correctly notice the time, date and place of certificate verification and be present for certificate verification at their own expense.

The date and time for Certificate Verification and interview for assessing suitability of PH candidates who have been included in the Probability List will be intimated later. Individual Memos will be sent to those candidates in the matter.

Documents to be produced for Certificate Verification

Date of Birth and age-The relevant page of SSLC or its equivalent Certificate with its original and true copy.
Educational Qualification-SSLC or its equivalent certificates in original and true copies.
Religion/Community- Community Certificate(for Communal reservation) for SC/ST and Non Creamy Layer Certificate to Other Backward Classes-Original and true copies.
Ex-serviceman- Discharge Certificate and Pension Payment Order in original with true copies.
PH Candidates- Medical Certificate from the Medical Board in original with true copies.
Candidates who are in Service- No Objection Certificate from the Head of Office in original.
Differences in name, date of birth and signature- The Gazette Notification for change of name, date of birth and signature with true copies.
